Reviewing 4 Cases – What Can We Learn?

History of Violations: Dollar General was fined again for putting workers at risk after facing earlier penalties totaling $3.3 million.
Fall Protection Violations: A contractor was cited after failing to provide lifesaving fall protection and training that resulted in a worker fatality.
General Duty Clause Violations: One of the nation's largest pork processors faces penalties for putting essential workers at risk by exposing them to repetitive motion injuries.
Process Safety Management Violations: A refinery was cited for failing to protect workers from hazardous chemicals following oil and vapor releases and fiery flaring incidents.
